Gallery Show and Tell Forms

Providing information about your artwork helps people to appreciate it more. We are providing some pdf forms you can fill out on your computer by typing information into the form’s input boxes and then printing out the form. On the new website we are including the title of the piece, the artist’s name, the materials used, and the finish when these are available. Including this information on the form, especially a title, will help the web master properly label your art on the gallery page.

If you have a single piece, you can use this form: Show and Tell Single eForm.

If you have two or more pieces, you can save paper by using this form: Show and Tell Double eForm.

The eForms can also be printed and filled out by hand. Please print, so the information is legible if you choose that method.
